Please enter your details so we can begin to process your claim. You need to complete the claim process within 30 days of your purchase date to be eligible for an O2 digital prepaid Mastercard.

It's important that you enter the order number exactly as it's been provided in your order confirmation email, store receipt or within your account details on the MyO2 app.

It's also crucial that you provide us with your correct contact details - we'll be contacting you using these throughout the process and to send your digital prepaid Mastercard to. Once you have entered details here, there will be no opportunity for you to amend them later.
